Our compassionate and effective on-site Nurse Practioner approach incorporates evidence-based medicine, emphasizes teamwork, communication and flexibility to deliver resident centered systems of care to improve quality of care for each resident at your facility.
Wound care management services are provided to Assisted Living, Long-Term Care and Home Health Care residents by our compassionate and wound certified Nurse Practitioners.
Weekly wound rounds are conducted at the bedside with the facility wound nurse or individually at the resident’s home.
Our treatment recommendations and documentations are consistent with the facility’s philosophy, protocols and formulary.
All medical encounter documentations are MDS 3.0 compliant.
Our Nurse Practitioners evaluate and manages all types of wounds and when necessary perform maintenance debridement to reduce bio-burden and potential infections.
Our multi-specialty approach ensures cost savings for the facility by preventing the need for the resident to be transported to outside clinics, offices, or wound care centers.
Our NP’s also provide ongoing education for the nursing staff and keep open lines of communication between the resident attending physician and family members.
We provide NP services for primary care and wound care solutions to patients at home in collaborations with their primary care physician (PCP). Our NPs will conduct initial assessments Upon discharge home from your facility and then, if needed and approved by PCP, follow up monthly and as needed.
This concierge service helps patients to avoid unnecessary trips to emergency rooms or hospitalizations and enhance management of the patients’ chronic condition in the comfort of their home.
Psychiatry Consulting Services: A trained and skilled Psyciatric Nurse Practitioner addresses regulatory compliance (F-319) in long-term Care Facilities and provides recommendations for wide range of mental health issues including: medication management(gradual dose reduction, medication to medication interactions). Our Psyciatric NPs also work in collaboration with your staff to improve the resident’s quality of life, decrease disruptive behaviors, enhance psychological wellbeing, and increase patient/family communication and satisfaction.
Physician Coverage Services: This service is specifically designed for our facility physicians who may want NP coverage at their medical offices, clinics, and Long-term Care Facilities while on leave of absence. Our NPs continue their practice without closing their offices or losing potential new patients, thus losing revenue.
Incontinence Consulting Services: A trained Nurse Practitioner addresses all residents with urinary incontinence issues through appropriate assessment, diagnosis and treatment regimen. Our program also targets those with (Urinary Tract Infections) and recommends steps to reduce future UTI’s. Our NPs provide recommendations that meet regulatory state compliance (F-315) and designed around as CMS initiatives and guidelines.
Protocol Consulting Services: We can provide or help design diagnosis specific protocol for your facility to address medical issues quickly and to improve patient's overall outcomes. We can also provide the facility with assessment and documentation tools to improve staff assessment and documentation skills.
